The Elder Care Continuum Helps You Understand Your Loved One's End-of-Life Journey

At the Elder Law Practice of Timothy L. Takacs, we use a model called the Elder Care Continuum to help families understand the natural progression of aging and its impact on a loved one's health, mobility, housing and financial resources.

Most traditional elder law attorneys, Medicaid planners and senior advisors won’t work with you until you’re facing the asset protection crisis created by the need for nursing home care.  Until the crisis, which typically happens late in the elder’s life, the burden of researching, coordinating and delivering care for the elder rests squarely on the shoulders of the primary caregivers.

Your first meeting with Tim Takacs and his team will focus on identifying your loved one's position on each resource line of the Elder Care Continuum and to identify gaps in care.  When your customized Life Care Plan is developed, it will define, organize, prioritize and mobilize every aspect of your loved one's care from the time the plan
is completed until the end of his or her life.
