The Johnson McGinnis Tennessee Senior Caregiver Award

Recognizing Exceptional Senior Care

Caregivers are an invaluable part of our society, offering not only their support and aid but friendship and meaningful connections. We invite you to nominate a full-time, working professional in the senior care industry who provides exceptional care. The recipient of this esteemed award will be announced during the annual Time Out Workshop. The recipient will receive a trophy and a $250 Visa gift card.

Nominate a Professional

Complete the form below. Nominators are asked to submit a brief form and a statement answering the question: "How Has This Caregiver Made a Positive Difference in the Life of a Senior they serve?" (250 Word Maximum).

Award ceremony will take place at the Time Out Workshop on Thursday, June 13, 2024 at The Inn at Opryland during lunch at 12:30 p.m. Winner and nominator must be present to win.

Reason for Nomination

Please submit a narrative explanation, of no more than 250 words answering the question "How Has This Caregiver Made a Positive Difference in the Lives of Others?" Any nominations longer than 250 words will NOT be considered.
